Clinical Usefulness of 99mTc-HMPAO brain SPECT in the Psychiatric Disability Evaluation of Patients with Traumatic Brain Injury


Abstract
Background :
@EN Accurate and objective assessment of permanent brain impairment is prerequisite for reasonable compensation in neuropsychiatric field. Functional imaging modality has potential for demonstrating abnormalities of brain not detectable by
traditional
morphological and electrophysiological imaging in patients with traumatic brain injury(TBI).
@ES Methods :
@EN Seventy patients with a remote history of TBI, visited for evaluation of psychiatric disability, were studied with conventional electroencephalography(EEG), magnetic resonance imaging(MRI), single-photon emission computed tomputed
tomography(SPECT)
using 99m-TcHMPAO, and clinical psychological test,
@ES Results :
@EN Overall, 56 patients(80.0%)showed regional cerebral blood flow deficits by HMPAO SPECT. Whereas 25 patients(35.7%) by EEC and 38 patients(54.2%) by MRI showed abnormal findings. Sensitivity of EEC, MRI, and SPECT, using psychological test as
the
criterion, were 0.39, 0.58 and 0.82 respectively, and overall predictive value of these techniques were 0.45, 0.61, and 0.78 This results implicate that HMPAO SPECT is more sensitive than EEG and MRI in the detection of abnormalities of the brain
in TBI
victims.
@ES Conclusion :
@EN In the evaluation of psychiatric disability for the patients with TBI, HMPAO brain SPECT is a useful technique to demonstrating evidence of brain dysfunction.
